1. Maternity Notification Form (MAT1 Form)

Online via the SSS website

No need to sumbit any forms or documents dahil napapaloob na sa iyong SSS online account ang lahat ny iyong detalye.

Sundin lamang ang mga hakbang na ito kung paano gumawa ng online Maternity Notification.

Via Self-service Express Terminal (SET) o SSS mobile app: (For Self-employed/ Voluntary/ OFW/ members separated by employment)

Napapaloob na sa SET o mobile app ang mga hakbang sa pag-sumite ng Maternity Notification.

Kailan dapat gagawin ang Maternity Notification?

Normally, nakokonpirma nating mga babae na tayo ay buntis aroung 4-6 weeks. Ito ay akma na para mag-submit ng Maternity Notification.

Hindi ka na pwedeng mag-submit ng Maternity Notification after ka nanganak. Kapag hindi mo alam na ikaw pala ay buntis, ikaw ay maituturing na Miscarriage case.

Pagkatapos manganak

• Maternity Benefit Application form (For Self-employed/ Voluntary/ OFW/ members separated by employment)
• Maternity Benefit Reimbursement Application form (For Employed members)
• Proof of Maternity Notification:
  • Maternity Notification na may tatak ng SSS na ito’y natanggap bago ang panganganak/ miscarriage
  • Maternity Notification confirmation kung nag-file ka via SET or SSS website
• Other main documents relating to mode of delivery and other personal circumstance. See list below.

Normal delivery

1. Certificate of Live Birth ng bata. Dapat ito ay registered sa Local Civil Registrar (LCS) or issued by Philippine Statistics Authority (PSA)
2. Report of Child’s Birth. Dapat ito ay notarized o authenticated ng Philippine Embassy or Consulate General, or apostilized ng Foreign Ministry or Embassy or Consulate General
3. Passport ng bata (if birth abroad)

Cesarean delivery

1. Certificate of Live Birth ng bata. Dapat ito ay registered sa Local Civil Registrar (LCS) or issued by Philippine Statistics Authority (PSA)
2. Report of Child’s Birth. Dapat ito ay notarized o authenticated ng Philippine Embassy or Consulate General, or apostilized ng Foreign Ministry or Embassy or Consulate General
3. Passport ng bata (if birth abroad)
4. Hospital records. Alinman sa sumusunod:
   • Operating Room Record
   • Surgical Memorandum
   • Discharge Summary Report
   • Medical/ Clinical Abstract
   • Delivery Report
   • Detailed invoice of costs of delivery (if birth abroad)
   • Other additional documents that may be required from you by SSS

Para sa panganganak na kalaunan ay nagresulta sa pagkamatay ng bata:

1. Certificate of Live Birth ng bata. Dapat ito ay registered sa Local Civil Registrar (LCR) or issued by Philippine Statistics Authority (PSA)
2. Report of Child’s Birth. Dapat ito ay notarized o authenticated ng Philippine Embassy or Consulate General, or apostilized ng Foreign Ministry or Embassy or Consulate General
3. Certificate of Death ng bata na registered sa Local Civil Registrar (LCR), or issued by Philippine Statistics Authority (PSA), or notarized/authenticated ng Philippine Embassy or Consulate General

Stillbirth

1. Fetal Certificate of Death na registed sa Local Civil Registrat (LCR), or issued by Philippine Statistics Authority (PSA), or notarized/ authenticated by Philippine Embassy/ Consulate General, or apostilized by Foreign Ministry/ Embassy/ Consulate General
2. Certified true copy ng hospital/ medical records na nagsasaad ng resulta ng panganganak

Miscarriage/ Pagkakunan

Kung nakapag-file na Maternity Notification, alinman sa sumusunod:

• Pregnancy test result
• Ultrasound report
• Histopathological report

Kapag hindi nakapag-file ng Maternity Notification OR kapag hindi mo alam na ikaw pala ay buntis, ikaw ay maituturing na Miscarriage case. You must submit the following documents:

1. Proof of pregnancy: (alinman sa sumusunod)
   • Pregnancy test result
   • Ultrasound report
   • Histopathological report
2. Pregnancy test result signed by a doctor or municipal health officer
3. Diagnostic test results
   • Ultrasound
   • Blood pregnancy test (Beta HCG)
   • Early pregnancy factor

Ectopic Pregnancy/ Hydatidiform Mole

1. Certified true copy of hospital/ medical records na nagsasaad ng resulta ng panganganak
2. Depende sa evaluation ng SSS, maaaring kakailanganin ang mga sumusunod:
   • Consultation records
   • Hospital Abstract/ Discharge summary
   • Medical/ Clinical Abstract

Kapag solo parent

• Solo Parent ID issued within 2 years since date of birth of child. Dapat ito ay issued by LGU at signed by SSS at may lagda ng Mayor kung saan ikaw nakatira.
• Kung walang Solo Parent ID, mag-submit ng Certificate of Eligibility ng Solo Parent. Ito ay issued by LGU at may lagda ng Social Worker/ Mayor.

Self-employed/ Voluntary na dating employed o mga myembro na separated from employment

If birth/miscarriage occurs within the employment period OR within 6 months from separation:

• Certificate of Separation from Employment. Dapat may petsa ng separation at impormasyon na walang paunang bayad na naibigay ang employer.

Kapag hindi makapagbigay ng Certificate of Separation from Employment:

• Affidavit of Undertaking. Dapat may effective date of separation at at impormasyon na walang paunang bayad na naibigay ang employer.
• Ang mga dahilan kung bakit hindi makapagbigay ng Certificate of Separation from Employment ay mga sumusunod:
  • Naka-strike ang kompanya.
  • Nabuwag o tumigil na sa operasyon ang kompanya.
  • May pending court case tungkol sa pagkahiwalay sa trabaho
  • Nahiwalay sa trabaho dahil sa Absence without offical leave (AWOL) o may hindi magandang relasyon sa employer
  • Member’s current address is more than 30 kilometers from employer’s address
  • Member has no records from previous employer anymore.
